Position: MSW Supervisor – PACE (Program of All-Inclusive Care for the Elderly)

Location: El Cajon, CA

Start Date: Immediate | Direct Hire

Schedule: Monday – Friday | 8:00 AM – 5:00 PM

Compensation: $39.93 – $51.67/hour (Dependent on Experience)

Key Responsibilities

Supervise and support social work staff in service delivery, compliance, and documentation.

Conduct and review psychosocial assessments, care plans, and interventions for PACE participants.

Perform home visits to assess living conditions and support participant safety and well-being.

Lead care coordination and multidisciplinary team meetings for complex cases.

Assist in training new staff and ensuring quality performance.

Utilize EPIC EMR and other platforms to maintain accurate and timely clinical documentation.

Maintain strong working knowledge of community resources and assist staff in utilizing them effectively.

Required Qualifications

Education: Master’s Degree in Social Work from an accredited institution – Required

Experience: Minimum 1 year of social work experience in a healthcare setting – Required

Experience with frail and/or elderly populations – Required

Certifications & Licenses:

Valid California Driver’s License

Reliable transportation with proper insurance to support frequent travel across San Diego County

First Aid/CPR Certification – or willingness to obtain

Skills:

Counseling theory and techniques

Organization and project management

Strong interpersonal and team leadership skills

Fluent in written and verbal English communication

Comfortable conducting in-home assessments

Preferred Qualifications

Previous experience in a PACE or community-based elder care program

Multilingual communication skills (Spanish or other languages)

Familiarity with California health regulations and elder care systems
