Job Summary:

Desert Canyon Post-Acute Care is seeking a dedicated and compassionate Occupational Therapist (OT) to join our dynamic rehabilitation team. The OT will be responsible for assessing, planning, and implementing individualized therapeutic programs to help our patients regain functional independence and improve their quality of life. The ideal candidate will have a strong understanding of geriatric care and a commitment to providing exceptional patient-centered therapy services.

Key Responsibilities:

  • Patient Assessment and Evaluation:
    • Conduct comprehensive evaluations of patients' physical, cognitive, and functional abilities.
    • Develop individualized treatment plans based on assessment findings and patient goals.
  • Therapeutic Intervention:
    • Implement and supervise therapeutic activities and exercises to improve patients’ daily living skills.
    • Utilize evidence-based practices and adaptive equipment to enhance patients' functional performance.
  • Collaboration and Communication:
    • Work closely with a multidisciplinary team, including physicians, nurses, physical therapists, and speech therapists, to ensure coordinated care.
    • Communicate effectively with patients, families, and caregivers about treatment goals and progress.
  • Documentation and Reporting:
    • Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
    • Report patient progress and any changes in condition to the healthcare team.
  • Patient and Family Education:
    • Educate patients and their families on home exercise programs, adaptive techniques, and safety measures to promote independence.
  • Professional Development:
    • Stay current with advancements in occupational therapy through continuing education and professional development opportunities.
    • Participate in in-service training and staff meetings.

Qualifications:

  • Education:
    • Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
    • Current Occupational Therapy license in the state of California.
  • Experience:
    • Experience in a post-acute or long-term care setting is preferred.
    • New graduates are welcome to apply.
  • Skills:
    • Strong clinical and problem-solving skills.
    • Excellent communication and interpersonal skills.
    • Ability to work independently and as part of a team.
    • Knowledge of electronic medical records (EMR) systems.
  • Certifications:
    • Basic Life Support (BLS) certification is required.
    • Certification from the National Board for Certification in Occupational Therapy (NBCOT) is preferred
