Job Description: Permanent Physical Therapist
Location: Capitola, CA, 95010
Facility: Skilled Nursing Facility (SNF)
Bed Count: 99
Shift: 40 hours per week, Monday to Friday
Weekend Rotation: None
Salary Range: Up to $60 per hour
Position Summary:
We are seeking a dedicated and compassionate Permanent Physical Therapist to join our skilled nursing facility in Capitola, CA. The ideal candidate will be responsible for evaluating and treating patients, helping them improve their mobility, and manage pain through physical therapy. This role is crucial in aiding our patients to achieve their highest functional potential and improving their overall quality of life.
Key Responsibilities:
- Patient Assessment and Evaluation: Perform comprehensive assessments to determine the nature and extent of physical therapy required.
- Treatment Planning: Develop individualized treatment plans tailored to each patient's specific needs and goals.
- Therapeutic Interventions: Implement and supervise appropriate therapeutic interventions to enhance strength, mobility, and function.
- Patient Education: Educate patients and their families about the therapy process, techniques, and exercises to aid recovery and improve daily living.
- Documentation: Maintain accurate and timely documentation of patient evaluations, progress, and treatment plans.
- Collaboration: Work closely with other healthcare professionals, including physicians, nurses, and occupational therapists, to provide a holistic approach to patient care.
- Compliance: Ensure all treatments are in accordance with federal, state, and facility policies and procedures.
Qualifications:
- Education: Master's or Doctorate degree in Physical Therapy from an accredited program.
- Licensure: Current and valid Physical Therapy license in the state of California.
- Experience: Prior experience in a skilled nursing facility is preferred but not required.
- Skills: Excellent communication, interpersonal, and organizational skills. Ability to work independently and as part of a team.
- Physical Requirements: Ability to perform tasks that require physical strength, such as lifting patients and equipment.
