Job Description
Position: Registered Nurse (RN) - Case Management
Location: Charleston, SC
Pay scale: $ 75per hour
Shift: Day
Responsibilities:
- Conduct comprehensive assessments to determine the healthcare needs of patients.
- Develop and implement individualized case management plans to ensure optimal patient care.
- Collaborate with healthcare professionals, social workers, and other stakeholders to coordinate and facilitate patient care.
- Monitor and evaluate patient progress and adjust care plans as needed.
- Advocate for patients to ensure access to appropriate resources and services.
- Maintain accurate and detailed documentation of case management activities.
Requirements:
- Registered Nurse (RN) licensure.
- Experience in case management or care coordination.
- Knowledge of healthcare resources, insurance, and reimbursement processes.
- Basic Life Support (BLS) certification.
