Key Responsibilities:

• Evaluate and Treat patients to improve mobility, strength, and overall physical function.
• Develop individualized treatment plans based on the patient’s specific needs and goals.
• Monitor patient progress and modify treatment plans as needed to ensure the best outcomes.
• Educate patients and their families on exercises, techniques, and strategies to support recovery and independence.
• Document patient progress accurately and comprehensively according to facility requirements.
• Collaborate with other healthcare professionals, including physicians, nurses, and other therapists, to deliver integrated care.

Qualifications:

• Education: Doctoral or Master's degree in Physical Therapy from an accredited program.
• Licensure: Current Physical Therapy license in California.
• Experience: Prior experience in a Skilled Nursing Facility (SNF) is preferred but not required.
• Skills: Excellent communication and interpersonal skills, with a compassionate, patient-centered approach to care.
