Key Responsibilities:
- Evaluate and Treat patients to improve mobility, strength, and overall physical function.
- Develop individualized treatment plans based on the patient’s specific needs and goals.
- Monitor patient progress and modify treatment plans as needed to ensure the best outcomes.
- Educate patients and their families on exercises, techniques, and strategies to support recovery and independence.
- Document patient progress accurately and comprehensively according to facility requirements.
- Collaborate with other healthcare professionals, including physicians, nurses, and other therapists, to deliver integrated care.
Qualifications:
- Education: Doctoral or Master's degree in Physical Therapy from an accredited program.
- Licensure: Current Physical Therapy license in California.
- Experience: Prior experience in a Skilled Nursing Facility (SNF) is preferred but not required.
- Skills: Excellent communication and interpersonal skills, with a compassionate, patient-centered approach to care.
