Job Description

Position: Physical Therapist (SNF)

Location: San Rafael, CA

Pay scale: $60 to $68 hourly on W2

Shift: Days

Responsibilities:

• Evaluate and Assess: Conduct thorough assessments of patients’ physical conditions, mobility, and functional abilities.
• Develop Treatment Plans: Create individualized treatment plans tailored to each patient’s needs and goals.
• Provide Therapies: Implement evidence-based physical therapy treatments to improve patients' strength, mobility, and quality of life.
• Collaborate: Work closely with interdisciplinary teams, including physicians, nurses, and other therapists, to ensure comprehensive patient care.
• Document: Maintain accurate, up-to-date patient records and progress notes in accordance with facility policies and regulatory standards.
• Educate: Teach patients and their families about home exercises and strategies to enhance recovery and improve independence.

Requirements:

• Doctorate or master's degree in physical therapy or equivalent.
• Specialized experience with specific patient populations or conditions may be necessary.
• Possession of a valid physical therapy license and relevant certifications.
