Job Description
Title: Registered Nurse - Case Manager
Shift: Day 5x8-Hour (08:00 - 16:30)
Duration: 13 Weeks
As a Registered Nurse Case Manager at our facility, you will be responsible for coordinating patient care to ensure proper treatment and follow-up. You will work closely with healthcare providers to deliver high-quality care and support to patients.
Duties
- Provide direct patient care and case management services
- Conduct patient assessments and develop individualized care plans
- Coordinate with healthcare team members to ensure comprehensive care delivery
- Assist in discharge planning and follow-up care arrangements
- Utilize medical terminology and knowledge of critical care practices
- Collaborate with various departments including the ER, PICU, and other units
- Maintain strict adherence to HIPAA regulations
Skills
- Strong background in patient care and case management
- Experience in a Level II Trauma Center is preferred
- Proficiency in conducting patient assessments and developing care plans
- Knowledge of medical terminology and critical care practices
- Ability to work effectively in a fast-paced environment
- Excellent communication and interpersonal skills
