Resident Care Manager

Job Description

Fall River Health Services is looking for a Resident Care Manager in partnership with the Director of Nursing is responsible to care plan, communicate, and manage resident care within Seven Sisters Living Center. Ensuring quality nursing care is provided and maintained according to provider orders and state/federal regulations. Supervises and mentors caregivers according to FRHS policy. Communicates with residents, families, and interdisciplinary team members ensuring resident care needs are known by each individual as required.The Residents Care Manager provides leadership, organization, planning and direction for optimum resident care which is responsive to the needs of the residents, consistent with the established standards of nursing practice, in line with quality and operating goals of Fall River Health Services.Supervisor Responsibilities:Supervise a team of Registered Nurses, Licensed Practical Nurses, Medication Aides, Nursing Assistants, and Health Unit Clerks in the long-term care setting.Manage staff by assigning and delegating tasks as neededParticipate in annual and “as needed” employee reviews. Provides constructive feedback on performance to meet organizational, personal and professional goalsAddress performance issues and offers corrective action when necessaryResponsibilities:Displays leadership and assume accountability to provide direction and coordination of nursing care and adequate staffing with cooperation from professional staff and the DON.Directly supervise and develop subordinates by direct teaching, coaching, guidance and by recognizing needs for staff development.Investigate/resolve complaints of subordinates, residents, visitors, ancillary departments, and medical staff through direct action or referral to the appropriate individuals as well as facilitating physician and staff interaction.Assess resident care, review current and past records, complete various data collection tools, review incident reports and work collaboratively with the DON and outside entities on Long Term Care Quality Improvement Initiatives.Collaborate with patient/family/staff/other health care disciplines in a professional, therapeutic and cooperative manner to contribute to a positive work environment and aid in health promotion, maintenance, restoration, and overall rehabilitation. Encourages staff development and professional growth and provides necessary support to the nursing staff to ensure staff is able to deliver quality care to the resident and meet his/her needs.Coordinate with other departments, ensuring all areas of the resident care are addressed on the MDS and care plan. Responsible for the completion of the entire RAI process (assessments, MDS, RAP’s, care plan, RN Coordinator signature for completion) following the state and federal regulations. Assures accurate data entry of RAI information into Electronic Health Record.Ensures all care practices are in compliance with federal, state, and local regulations as well as facility’s policies and procedures.Conducts regular assessments of care services and implements improvements as neededParticipate in quality improvement initiatives and programs to continuously enhance resident outcomesResponds to state survey findings in a proactive, positive and problem solving manner, deficiencies are addressed through process, system reviews and/or direct intervention to ensure compliance.Coordinate and track competencies of SSLC Care Team on reoccurring basis.Responsible for other duties as assigned on occasion, based upon facility needs or requirements.Education/Experience/Licensure: Education/Experience: Bachelor’s degree in nursing from an accredited college or university preferred, or be a graduate of an approved RN program. Must have thorough nursing care experience to include direct patient care, with previous management responsibilities Technical Skills: The incumbent must possess a broad understanding of effective managerial, communication, and leadership skills. Other qualifications/certificate/license: The incumbent must possess a current, unencumbered professional nurse’s license from the State of South Dakota. Fall River Health Services (FRHS) is a comprehensive healthcare system, which services Fall River County and the surrounding areas in the beautiful Black Hills of South Dakota.
