Job Summary: We are seeking a dedicated and experienced Physical Therapist to join our team at Ellison John Transitional Care Center in Lancaster, CA. The ideal candidate will be passionate about helping patients achieve their rehabilitation goals, providing individualized treatment plans, and working collaboratively with a multidisciplinary team.

Responsibilities:

  • Conduct comprehensive assessments of patients’ physical conditions and functional abilities.
  • Develop and implement personalized treatment plans to improve patients' mobility, strength, and overall physical function.
  • Use various therapeutic techniques and modalities to aid in patients' recovery.
  • Monitor and document patients’ progress, adjusting treatment plans as needed.
  • Educate patients and their families on home exercise programs, safety precautions, and ongoing care.
  • Work closely with other healthcare professionals, including doctors, nurses, and occupational therapists, to ensure coordinated patient care.
  • Maintain accurate and timely patient records in compliance with regulatory standards.
  • Participate in continuous professional development and stay current with advancements in physical therapy practices.

Qualifications:

  • Doctor of Physical Therapy (DPT) degree from an accredited institution.
  • Valid California state license to practice as a Physical Therapist.
  • Previous experience in a transitional care or similar setting is preferred.
  • Excellent interpersonal and communication skills.
  • Strong organizational skills and attention to detail.
  • Ability to work effectively both independently and as part of a team.
  • Commitment to providing high-quality, compassionate care.
