Locum - Physician - Anesthesiology - Cardiac Phoenix AZ

Job Description

Summary: SJHMC is in need of 2 fulltime cardiac locums. 

Coverage-Cath Lab and CV OR rooms. Call shifts are 24 hours with 10-12 hours per day in house, the rest of the shift is at home. Full time is 4 shifts per week (M-F) and one weekend a month. Weekend is home call with coming in only for cases as needed. Volume is location and case specific but the hours are generally 10-12 per day. Hours are 7am to 5 pm. After 5, they are at home and on-call.

Rate Caps for Submission:

• Malpractice – Must be included in rates.  
• Orientation – $275 - half the hourly/callback rate, kicks in after 8 hours. “Per hour of orientation or training beyond 8 hours” 
• Hourly – $550 - “Per hour, not OT”
• Holiday – Must be 1.5x the regular rate. “Per holiday hour, no OT”
• On Call (Daily) – $4500 - “Per 24 hours to include 4 hours gratis'  
• 24 Hour Holiday Call Rate - $6750 - “Per 24 hours to include 4 hours gratis' 
• Call Back – $550 – “Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.” 
• Holiday Call Back – $825 - “Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.”

Facility Location: St. Joseph's Hospital and Medical Center

Setting: Cath Lab and CV OR

FTE: 4 FTE's (equivalent of part time must equal 4)

Duties:  Provide anesthesia coverage for Cardiovascular Services

 Required Procedures: TEE and central lines

EMR: Cerner

Support Staff: 

Reason for Coverage: Ending of contract for anesthesia services-Transition to employment model that is 90+ days out

Deal breakers (do not present this): Have to have AZ license and clean file; must have cardiac experience

Credentialing Information:

Timeframe: Immediate (Emergency Privs possible)
