Job Description: Physical Therapist(Home Health)

Location: Davison, MI 48423

Shift: 8:00 AM - 4:30 PM

Pay: $60 to $65 per hour (W2)

Job Overview:

We are seeking a dedicated and skilled Physical Therapist to join our Home Health team in Davison, MI. The ideal candidate will provide high-quality care to patients in their homes, helping them improve their mobility and manage pain. This role is essential in assisting patients to recover from injury or illness and enhancing their quality of life.

Key Responsibilities:

  • Conduct thorough assessments of patients' physical abilities and limitations.
  • Develop personalized treatment plans based on patient evaluations and goals.
  • Implement physical therapy interventions, including exercises, manual therapy, and use of equipment.
  • Educate patients and their families on treatment plans, exercises, and safety measures.
  • Monitor and document patients' progress, modifying treatment plans as needed.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Provide guidance and support to patients in managing chronic conditions and preventing further injury.
  • Maintain accurate and timely records in compliance with regulations and company policies.
  • Travel to patients' homes within the Pittsfield area to deliver therapy services.
  • Ensure all services provided comply with state and federal regulations and company standards.

Qualifications:

  • Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
  • Valid and current state license as a Physical Therapist in MI.
  • Minimum of [insert number] years of experience in physical therapy, preferably in a home health setting.
  • BLS and First Aid certification.
  • Strong interpersonal and communication skills.
  • Ability to work independently and manage time effectively.
  • Valid driver's license and reliable transportation.
