Job Description

 

Facility –  Riverside Community Hospital 

Start Date-asap

Cost Center - 93327

Unit – Case Management  ****MUST HAVE RECENT ACUTE CASE MANAGEMENT EXPERIENCE*****

Shift – 8:00AM-5:00PM

Assignment Length – 13 Weeks

 

 

 Requested Skill Set:

Must have: Current RN License, CCM preferred, Bachelor’s Degree preferred, minimum of one year acute care or payer case management, utilization management, discharge planning, resource management experience required.  Minimum of three years of acute care nursing required.  Proficiency with information systems and computer systems; MIDAS preferred.    

Advanced skills needed:  understanding of utilization management, case management, health care financing, alternative care options, goals, outcomes, regulatory and professional implications of practice. 

Procedures performed on unit:  assessing, planning, facilitating, and advocating for options and services through the continuum of care from point of contact through discharge on assigned patients.  Case Manager promotes quality and cost effective outcomes.  Emphasis is on care coordination, communication, and collaboration with nursing, physician, departments within the facility, insurers and post-acute service providers to pace the care toward optimal outcomes within the appropriate level of care.  The Case Manager consistently conducts the utilization review process on those patients who require it, and the discharge planning process as required by hospital policies, standards of practice, and Federal and State regulations.
