•         Number of Beds - 36

•         Number of Staff – 2

•         Type of staff:

o    Day Shift – 1 RN and 1 SW

o    Night Shift – no CM on nights

•         Patient Ratios – 1:25 RN

•         Type of equipment - none

•         EMR – Meditech and Midas

Must have:  candidates without these skills will not be considered for the role.

•         HCA Case Management experience

•         Meditech and Midas experience

Preferred or nice to have:  candidates with these skills will be considered first.

•          Discharge planning

•          Patient progression
