Job Description

Position:  LVN(Home Health)/Licensed Vocational Nurse (LVN)

Location: 

Lima, OH, 45804

Pay scale: $40 - $42/hr on w2 without benefits

Shift: 8AM - 4:30 PM

Weekend Requirements: Every Third Weekend

Key Responsibilities:

• Provide direct patient care in a home setting.
• Administer medications and treatments as prescribed.
• Monitor and record patient vital signs.
• Assist with activities of daily living (ADLs).
• Educate patients and their families on health care needs and safety.
• Collaborate with the healthcare team to develop and implement care plans.

Qualifications:

• Current LVN license in the state of Ohio.
• Active BLS certification.
• Previous experience in home health is preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.

Requirements:

• Completed vocational nursing program.
• COVID Vaccination/ Valid CPR/ Health Documents/ License/ Physical
• Updated Resume
