Job Summary: We are seeking a dedicated and experienced Physical Therapist to join our team at Interface Rehab in Placentia , CA. The ideal candidate will be passionate about helping patients achieve their rehabilitation goals, providing individualized treatment plans, and working collaboratively with a multidisciplinary team.

Responsibilities:

• Conduct comprehensive assessments of patients’ physical conditions and functional abilities.
• Develop and implement personalized treatment plans to improve patients' mobility, strength, and overall physical function.
• Use various therapeutic techniques and modalities to aid in patients' recovery.
• Monitor and document patients’ progress, adjusting treatment plans as needed.
• Educate patients and their families on home exercise programs, safety precautions, and ongoing care.
• Work closely with other healthcare professionals, including doctors, nurses, and occupational therapists, to ensure coordinated patient care.
• Maintain accurate and timely patient records in compliance with regulatory standards.
• Participate in continuous professional development and stay current with advancements in physical therapy practices.

Qualifications:

• Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Valid California state license to practice as a Physical Therapist.
• Previous experience in a transitional care or similar setting is preferred.
• Excellent interpersonal and communication skills.
• Strong organizational skills and attention to detail.
• Ability to work effectively both independently and as part of a team.
