Key Responsibilities:

• Evaluate patients' physical, emotional, and psychological needs to develop personalized occupational therapy plans.
• Implement therapeutic activities and exercises to promote patient independence and improve daily living skills.
• Monitor patient progress, making adjustments to treatment plans as necessary, and report findings to the healthcare team.
• Educate patients and families on techniques to improve daily functioning, mobility, and self-care abilities.
• Collaborate with other healthcare professionals to ensure cohesive, holistic care.
• Document patient progress accurately and maintain compliance with SNF documentation standards.

Qualifications:

• Education: Graduate of an accredited Occupational Therapy program.
• Licensure: Valid California Occupational Therapist license.
• Experience: Previous experience in a Skilled Nursing Facility (SNF) setting is preferred, but not required.
• Skills: Strong communication and interpersonal skills with a patient-centered approach to care.
