Blanchard Valley Health System in Northwest Ohio is seeking a compassionate and experienced Nurse Practitioner with a strong commitment to palliative care, hospice, and geriatrics. Join a community-based nonprofit that truly values its providers and their impact on patient lives.

• This position is for INPATIENT care.
• Deliver care in a 75% inpatient / 25% outpatient setting
• Provide palliative care services primarily at Lima Memorial Hospital
• Work Monday–Friday, 8:00 AM–5:00 PM, managing approximately 6 patients per day
• Practice autonomously to manage symptoms and chronic diseases
• Lead goals of care discussions and provide advanced care planning
• Round on inpatient hospice patients as part of daily duties
• Collaborate with hospitalists, who manage patient admissions
• Function within an open ICU environment
• Participate in a rotating on-call schedule
• Role requires experience as an advanced provider in acute care, palliative care, or hospice
• Ideal candidates will have a strong passion for palliative care, hospice, and geriatrics

• Not accepting new graduate candidates - experience required

Requirement description :

• Board Certified NP - REQUIRED
• OH license (or willing to obtain) - REQUIRED
• Experience as advanced provider in an acute care setting or with palliative care and hospice (NO EXCEPTIONS) - REQUIRED
• Clean malpractice and license - REQUIRED
• Date of agency's last communication with provider (put answer in 'Additional Submission Details') - REQUIRED
