Job Description: Physical Therapist Assistant (PTA) - Home Health (10-Week Contract)
Location: New Lenox, IL
Shift: Days
Hourly Pay: $30 to $32
Contract Duration: 10 Weeks
Position Overview: We are seeking a dedicated and experienced Physical Therapist Assistant (PTA) to join our Home Health team in New Lenox, IL, for a 10-week contract. This role is perfect for a skilled PTA looking to make a significant impact in the lives of patients by providing high-quality, compassionate care in their homes.
Key Responsibilities:
- Provide physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist.
- Assist in developing and implementing individualized treatment plans aimed at improving mobility, relieving pain, and preventing or limiting physical disabilities.
- Monitor patient progress and document treatments, responses, and outcomes.
- Educate patients and their families on continued physical therapy at home to promote overall wellness and recovery.
- Collaborate with the healthcare team to ensure seamless care coordination and optimal patient outcomes.
Qualifications:
- Experience: Minimum of 1 year of Home Health PTA experience - REQUIRED
- Licensure: Must hold a valid Physical Therapist Assistant license in the state of Illinois.

Certifications:
- Basic Life Support (BLS) certification from the American Heart Association (AHA) - REQUIRED
- COVID-19 vaccination - REQUIRED
- Transportation: Valid driver's license and car insurance - REQUIRED

- Ability to work independently while being part of a larger healthcare team.
- Adaptability to a dynamic work environment.
- Proficiency in documenting patient care and progress.
