Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Home Health

Location: Lima, OH 45804

Contract Duration: 13 Weeks

Shift: 8:00 AM - 4:30 PM

Hourly Pay: $30 to $35 (W2)

Experience Required:

• Minimum of 3 years of Home Health experience
• HomeCare HomeBase EMR experience
• Basic Life Support (BLS) certification (American Heart Association - AHA)

Job Description:

Position Overview: We are seeking a dedicated and experienced Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) for a 13-week contract position in Lima, OH. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring comfort and promoting their overall well-being. This role requires strong clinical skills, excellent communication, and a compassionate approach to patient care.

Key Responsibilities:

• Provide direct nursing care to patients in their homes as per the physician’s plan of care.
• Administer medications, wound care, and other treatments as prescribed.
• Monitor patient conditions, identify changes, and report to the supervising nurse or physician.
• Educate patients and their families on health care and treatment plans.
• Utilize the HomeCare HomeBase EMR system to document patient care and update medical records accurately and promptly.
• Perform regular patient assessments and participate in care plan development and revisions.
• Maintain patient confidentiality and comply with HIPAA regulations.
• Ensure all care provided is in accordance with state and federal regulations and professional nursing standards.
• Collaborate with the healthcare team to ensure comprehensive and coordinated patient care.

Qualifications:

• Active LVN/LPN license in the state of Ohio.
• Minimum of 3 years of experience in home health care.
• Proficiency in using HomeCare HomeBase EMR system.
• Current Basic Life Support (BLS) certification through the American Heart Association (AHA).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Reliable transportation and willingness to travel to patient homes within the designated area.

Physical Requirements:

• Ability to lift, move, and transfer patients as needed.
• Frequent standing, walking, bending, and reaching.
• Ability to work in a variety of home environments and conditions
