Position: LVN/LPN - Long Term Care (Nights)
Location: Kane, PA 16735
Duration: Perm
Pay rate: $25/Hr
Shift: 6:00 PM - 6:00 AM
Shift Notes:
Nights 6PM - 6AM // Two days on // Two days off // Three days on // 84-hours biweekly pay period
Position Overview: The Licensed Practical Nurse (LPN) plays a vital role in providing high-quality care to residents while ensuring compliance with established procedures and protocols. This position requires effective collaboration with nursing assistants, registered nurses, physicians, and other healthcare team members.
Key Responsibilities:
- Administer and document medications, treatments, and glucose checks in compliance with physician orders, reporting resident responses as needed.
- Accurately transcribe physician orders and update resident care plans to reflect current conditions.
- Collaborate with nursing assistants during monthly summary reviews and care plan updates.
- Notify RN Supervisor of significant changes in residents' conditions, and report any status changes.
- Coordinate resident transfers, notifying family members and department heads, and completing necessary transfer forms.
- Assist physicians during rounds and treatments, and collect lab specimens, ensuring accurate documentation and timely delivery.
- Oversee post-mortem procedures, notify department heads, and assist with resident belongings.
- Encourage resident participation in scheduled activities and communicate care plan changes to the team.
- Conduct performance evaluations of nursing assistants and document incident reports as needed.
- Record and monitor residents’ vital signs, including reweighing residents with weight discrepancies.
- Ensure the cleanliness and organization of work areas, treatment trays, and resident belongings.
- Supervise nursing assistants to ensure the care plan is carried out effectively.
- Participate in team conferences, policy updates, census management, and quality assurance studies.
- Provide direct resident care when required, following infection control practices.
- Attend staff meetings, in-service training, and other relevant meetings.
Requirements:
- BLS (AHA) certification (required).
- Nursing license in PA or temporary work permit (required at interview).
- Knowledge of medical asepsis, personal care of residents, nursing theory, and equipment use.
- Willingness to comply with institutional policies and regulations.
- Strong communication, observation, and teamwork skills.
Preferred:
- Nursing home experience (preferred).
