Key Responsibilities:

• Evaluate patients to assess their functional abilities and develop individualized treatment plans.
• Implement therapeutic interventions and activities designed to improve patient independence.
• Monitor patient progress and adjust treatment plans as necessary to meet goals.
• Educate patients and their families on techniques to promote long-term health and functional independence.
• Document patient care and progress with accuracy in compliance with facility standards.
• Collaborate with other healthcare professionals to ensure coordinated and effective patient care.

Qualifications:

• Education: Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
• Licensure: Valid Occupational Therapist (OT) license in California.
• Experience: Experience in a skilled nursing facility (SNF) setting is preferred but not required.
• Skills: Excellent communication skills, with a compassionate and patient-centered approach to care.
