Position Overview:

We are seeking an experienced Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) for a 13-week contract role in Home Health in Santa Maria, CA. The ideal candidate will have a strong background in home health care, with proficiency in Cerner and the ability to manage a varied patient load. This role requires flexibility and a commitment to providing high-quality care to patients in their homes.

Key Responsibilities:

• Provide direct patient care in a home health setting, managing a patient load of approximately 5 visits per day.
• Conduct patient assessments, develop care plans, and deliver nursing care in accordance with established protocols.
• Utilize the Cerner EMR system to document patient care and update medical records accurately.
• Perform infusions as needed, primarily for adult patients, with the understanding that pediatric patients requiring infusions will be referred to the ER for evaluation.
• Participate in the weekend rotation, working every 3rd weekend, with the option to take a day off the week before and after if desired.
• Adhere to the holiday rotation schedule, ensuring coverage during holidays as required.
• Be on-call 2-4 times per month during weekdays from 5 PM to 8 AM.
• Complete a 1.5 to 2-week floor-specific orientation to become familiar with the home health unit's operations.

Qualifications:

• Current LPN/LVN license.
• Minimum of 2 years of experience in home health care or a related post-acute care setting.
• Proficiency with Cerner and Netsmart (formerly known as Allscripts) EMR systems.
• Strong organizational and time-management skills, with the ability to manage a diverse patient load.
• BLS certification is required.
• Flexibility to work 8-hour shifts, typically Monday through Friday from 8 AM to 5 PM, with a 1-hour lunch break.
