Job Description: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Position Type: Permanent, Full-Time
Shift: Night Shift, 6:00 PM - 6:00 AM
Shift Schedule: Two days on // Two days off // Three days on
Biweekly Hours: 84 hours
Hourly Pay Rate: $25.00 (Plus $0.75 Night Shift Differential)
Position Overview:
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to join our Long Term Care (LTC) team in Kane, PA. This position offers a stable night shift schedule with opportunities to work closely with residents, nursing assistants, and high school students. The ideal candidate will have a strong commitment to providing high-quality care and a passion for interacting with residents daily.
Key Responsibilities:
- Carry out the medical plan of care under the direction of the RN Supervisor.
- Supervise Nursing Assistants to ensure resident care is provided according to good nursing practices and applicable State and Federal regulations.
- Administer and record medications, treatments, and monitor resident conditions (e.g., glucose checks).
- Accurately transcribe physician orders and complete monthly summaries.
- Review and update care plans in collaboration with Nursing Assistants to reflect the resident’s current condition.
- Notify the RN Supervisor of significant changes in resident conditions and document accordingly.
- Communicate resident transfers to immediate family and department heads, complete transfer forms, and assist with the transfer process.
- Assist physicians during rounds, examinations, and treatments as needed.
- Collect laboratory specimens (e.g., urine, cultures, stool) and ensure timely delivery with complete requisitions.
- Supervise and assist with post-mortem procedures and notify department heads of resident death.
- Encourage resident participation in scheduled activities.
- Provide updates to the care team on resident conditions, care plan changes, and special assignments.
- Assist with annual evaluations of Nursing Assistants, write commendations, and counseling records as needed.
- Accurately complete incident reports and document weights and vital signs, including reweighing residents when necessary.
- Clean and stock treatment trays, maintain work area cleanliness, and submit maintenance requests as needed.
- Supervise and direct resident care by Nursing Assistants, ensuring compliance with prescribed care plans.
- Attend care plan conferences, in-services, department meetings, and committee meetings as required.
- Assist in the orientation of new nursing staff, conduct team conferences, and monitor nurse aide job performance for infection control compliance.
- Provide direct resident care as needed and assist Certified Nursing Assistants (CNAs) on the hall.
Qualifications:
- Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) certification.

Compensation:
- Hourly Pay Rate: $25.00, with an additional $0.75 Night Shift Differential starting at the beginning of the shift.
