Position: Social Worker - Perm
Location: Lincoln Park, MI
Pay scale: Upto $22 hourly
Shift: 10hr shift
Responsibilities:
- Respond to crisis calls during and after business hours (as scheduled):
- Answer calls to designated on-call crisis tablet and/or phone immediately;
- If a call is missed, call back within 5 minutes;
- Conduct crisis screening protocols via telehealth to assess safety, deescalate the situation, determine immediate service needs and level of care;
- Consult with police and/or other emergency personnel, guardian and/or family members regarding safety planning, recommendations for intervention and aftercare;
- Consult with crisis supervisor to review recommendations and confirm plan for referral to least restrictive environment;
- As clinically indicated:
- Facilitate referral to inpatient psychiatric facility;
- Facilitate referral to COPE as needed;
- Facilitate same-day or next-day screening for enrollment in community mental health and/or SUD treatment services via the DWIHN access center;
- Facilitate referral for same-day, next-day or next-available appointment with their assigned or preferred CMH provider for aftercare;
- Facilitate referrals for community services, i.e., shelter, food bank, FQHC or primary care provider, health department, support group, etc.
- Complete necessary consents and releases of information to facilitate aftercare arrangements.
- Place follow-up phone calls to clients, guardians and/or family members and responsible providers the next day and subsequent days as needed to coordinate and confirm that aftercare services have been received;
- Document all contacts per established guidelines;
- Debrief with crisis supervisor and other members of the treatment team within 24 hours.

Assigned TGC cases:
- Conduct screenings and comprehensive assessments to identify areas of need and service preferences based on information from all available sources;
- Provide clients, guardians and/or family members with complete and accurate information regarding services and community resources available to meet the identified needs;
- Assist clients in determining their eligibility for entitlements, community services and supports;
- Coordinate with treatment team members and other providers, including representatives from state and community hospitals to facilitate successful transition to less intensive services in the community;
- Facilitate access to aftercare appointments following discharge from inpatient and residential treatment settings;
- Provide transportation assistance as needed until the client is able to access transportation resources independently;
- Develop Individualized Plans of Service with clients and other members of their treatment teams;
- Provide face-to-face in-person office and/or home visits and/or telehealth services to each assigned client based on their needs and preferences and according to the frequency outlined in their IPOS;
- Conduct community outreach as needed in order to facilitate client engagement or reengagement and to ensure access to needed services and supports;
- Provide crisis intervention, crisis and safety planning as clinically appropriate.
- Establish and maintain collaborative relationships with local providers;
- Participate in regularly scheduled clinical supervision, team meetings, case consultations and huddles as scheduled;
- Document all service activities and phone contacts within expected timeframes;
- Perform within acceptable standards of productivity;
- Maintain appropriate state licensure and certification;
- Make referrals to Recipient Rights, Adult and Child Protective Services as required;
- Complete 24 hours of program and/or position specific training annually;
- Follow the center’s professional code of ethics;
- Abide by all of the center's policies and procedures;
- Other duties as assigned.
Requirements:
- Candidates for the Crisis Therapist position must have a minimum of a Master’s degree in Social Work, Psychology or Counseling and possess the corresponding State of Michigan licensure. Candidates without a state license can be considered but must obtain state licensure (LLMSW, LLPC, TLLP, LMSW, LPC or LLP) within a 60-day training period beginning on the date of hire. Failure to obtain licensure within the training period will result in the employee being unqualified to perform work functions. Beyond the training period, all state licensure must continually remain active, in effect, and free from disciplinary action.
- Must possess a valid driver’s license and a good driving record, and obtain a Michigan Chauffeur License within 30 days of employment.
- Candidates must obtain the Qualified Mental Health Professional and Substance Abuse Treatment Specialist (SATS) credentials within 120 days of hire.
- Candidates must possess a CAADC credential, or be on an active, approved registered MCBAP certification Development Plan within six weeks of hire. Upon hire, the application must be submitted for the Development Plan to MCBAP, with or without the required trainings, within 30 days of hire, or they will not be permitted to provide services. Documentation of required trainings must be submitted with the Development Plan Application or within 90 days of hire. Candidates must have one year of experience (may include time spent in fieldwork or internship) treating or working with adults who have been diagnosed with serious mental illnesses.
