Job Description: Physical Therapist (Home Health)

Location: New Lenox, IL 60451

Duration: 13 weeks

Shift: 8:00 AM - 4:30 PM (40 hours guaranteed)

Pay: $50 to $55 hourly on W2

Position Overview:

We are seeking a dedicated and experienced Physical Therapist to join our Home Health team in New Lenox, IL. This role involves providing high-quality physical therapy services to patients in their homes, helping them to improve their mobility and overall quality of life. The ideal candidate will be compassionate, detail-oriented, and capable of working independently.

Key Responsibilities:

• Patient Assessment: Conduct comprehensive assessments of patients' physical condition, identifying functional limitations and determining appropriate therapeutic interventions.
• Care Planning: Develop individualized care plans based on patients' needs, goals, and physician recommendations.
• Therapeutic Interventions: Implement and oversee physical therapy treatments, including exercises, manual therapy, and modalities to improve patient mobility, strength, and function.
• Patient Education: Educate patients and their families on treatment plans, exercises, and lifestyle modifications to support recovery and prevent further injury.
• Progress Monitoring: Regularly evaluate and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Collaboration: Work closely with physicians, nurses, and other healthcare team members to ensure coordinated and effective patient care.
• Compliance: Ensure all services are delivered in accordance with federal, state, and local regulations, as well as professional standards of practice.

Qualifications:

• Education: Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
• Licensure: Current and valid state licensure as a Physical Therapist in Illinois.
• Experience: Minimum of 1-2 years of experience in physical therapy, preferably in a home health setting.
• Certifications: BLS certification required; additional relevant certifications are a plus.
• Skills:
  • Strong clinical skills in physical therapy assessment and intervention.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Proficient in electronic medical records (EMR) and other documentation systems.
