Job Summary

We are seeking a motivated and skilled Home Health RN to join our team. The ideal candidate will deliver high-quality nursing care to patients in their homes, promoting comfort, safety, and well-being. This role requires strong clinical skills, adaptability, and a commitment to patient-centered care.

Duties

• Conduct comprehensive patient assessments, develop and implement individualized care plans.
• Provide direct nursing care, including wound care, medication administration, and patient education.
• Monitor patients’ conditions and communicate updates to the healthcare team as needed.
• Document all care and procedures accurately in electronic health record systems, such as Epic or Meditech.
• Coordinate with other healthcare professionals to ensure comprehensive patient care.
• Adhere to infection control protocols and maintain patient safety standards.
• Educate patients and families on self-care and preventive measures to support continued well-being.

Experience

• Minimum 2 years of experience as an RN, ideally with experience in home health or community health settings.
• Must have OASIS (Outcome and Assessment Information Set) Experience
• Home Health and Hospice Experience required
• Experience traveling from patient to patient for home visits
• Must be able to manage and adjust to changes in schedule
• Must have drivers license in good standing

Requirements

• Active Vermont RN license or Compact license.
• BLS (Basic Life Support) certification required.
• This position is ideal for an independent and detail-oriented nurse who values patient-centered care and flexibility.

Travel Housing Rate

For travelers, housing costs in the Newport, VT area typically range from $1,200 to $1,700 per month for shared apartments or houses, offering affordable options during your assignment.
