o   Day Shift – 9 Clinical Pharmacists (2 in central, 7 decentralized), 9 Pharmacy Technicians (2 in IV room, 3 Pyxis Delivery, 1 Runner, 1 Central)

o   Night Shift – 2 Clinical Pharmacists (2 in central), 2 Pharmacy Technicians (1 in central, 1 in IV room)

•Patient Ratios – N/A

•Type of equipment – PyxisES, DoseEdge, KitCheck, Vigilanz, OnBase, Medex

•EMR - Meditech

•Vancomycin, aminoglycoside, warfarin, heparin dosing

•Medication order verifications

•IV product verifications

•Renal Dose adjustments, IV to PO Conversions

Preferred or nice to have:  candidates with these skills will be considered first.

•Critical Care Experience

•NICU Experience

•Pharmacy Residency

•Board Certification

•Medication Order Verification

•Medication Dosing Consults (Vancomycin, Aminoglycosides, Warfarin, enoxaparin, acetylcysteine)

•Antimicrobial stewardship, opioid stewardship

•Working through automatic protocols such as IV to PO conversations, Renal Dosing
