Patient Population:  Adult and Geriatric

EMR:  Crown Web

Dress Code: Dialyze Direct will provide black scrub tops.  Scrub bottoms: Personal scrubs of any color

Non-billable orientation: 8hrs

Holiday and OT Multipliers: 1.25%

Call, Call-Back and Charge:  Not applicable for this need

Requirement description :

• 2 years or more CURRENT Dialysis experience - Required 
• Copy of Current, Valid Driver License - Required
• Geriatric patient experience - Required
• NXStage Dialysis Machine experience Required  -    Please note experience on submission
• CNN or CDN Certification - PREFERRED 
• Crown Web EMR experience - Preferred
• BSN - Preferred

• Work History
• Skills Checklist (pertinent to position), current to 1 year 
• 1 Supervisory Signed Reference (from within the past two years of employment)
• License/Certifications - Must be uploaded with profile 

CERTIFICATION REQUIREMENTS :

• BLS (AHA)

STATE LICENSE REQUIREMENTS :

• Ohio

ADDITIONAL LICENSE REQUIREMENTS : Must be in hand upon submission

Weekend Requirements : None/ Holidays as needed
