Job Description

Position: Physical Therapist (SNF)

Location: Modesto, CA

Pay scale: $60 to $68 hourly on W2

Shift: Days

Responsibilities:

• - Conduct comprehensive assessments of patients’ physical conditions and functional abilities.
• - Develop and implement individualized treatment plans to enhance mobility, strength, and overall quality of life.
• - Collaborate closely with a multidisciplinary team, including physicians, nurses, occupational therapists, and speech therapists.
• - Educate patients and their families on exercises and techniques to support recovery and promote independence.
• - Maintain detailed and timely documentation of patient progress, treatment interventions, and outcomes.
• - Participate in team meetings and engage in continuing education to stay current with best practices in physical therapy.

Requirements:

• - Doctorate or Master’s degree in Physical Therapy from an accredited program.
• - Valid California Physical Therapy license.
• - Experience in a skilled nursing facility preferred, but not required.
• - Excellent communication and interpersonal skills.
• - Compassionate and patient-centered approach to care.
