Key Responsibilities:

• Develop and Implement personalized treatment plans to improve patient independence and quality of life.
• Provide Therapeutic Interventions to support patients in regaining physical, emotional, and social function.
• Educate patients, families, and caregivers on techniques to enhance recovery and promote autonomy.
• Document patient care plans and progress accurately.
• Collaborate with the interdisciplinary team to achieve optimal care outcomes.

Qualifications:

• Education: Bachelor’s or Master’s degree in Occupational Therapy from an accredited institution.
• Licensure: Valid California OT license.
• Experience: Previous SNF experience preferred, but not required.
• Skills: Strong communication and interpersonal skills, with a compassionate, patient-centered approach.
