RNs record patients’ medical histories and symptoms, help perform diagnostic tests and analyze results, operate medical machinery, administer treatment and medications, and help with cases follow up and rehabilitation. The Registered Nurse (RN) utilizes the nursing process, critical thinking and problem solving skills to provide holistic care to individuals, families, groups, communities and populations across the life span. As a Charge RN, the primary role is to ensure that all nursing functions within the department run smoothly and efficiently. They also provide guidance and support to help staff through challenging situations as they arise. Delegates assignments and provides supervision for the support staff in accordance with their level of training and the patient's acuity. Works, in cooperation with medical providers to ensure the quality of patient care. Maintains competency in all organizational, departmental, and outside agency environmental, employee, or patient safety standards relevant to job performance. • Maintains competency in all organizational, departmental, and outside agency environmental, employee, or patient safety standards relevant to job performance. o Assessment: § Provide initial and ongoing assessments and interpretation of patients or clients within the framework of holistic professional nursing practice, and in accordance with the principles of patient and family centered care as well as in accordance with facility policies, procedures, or protocols. § Assesses and interprets information and data regarding the patient or client’s culture and values system, lifestyle, and interest and ability to participate in their care, and other factors impacting successful treatment outcomes. § Assesses and interprets patient or client’s ability (physical, social and psychological) to manage their health in order to transition their care or discharge. In collaboration with the patient or family identifies patient or family needs based on analysis of data. Identifies and communicates patient or family needs to the interdisciplinary health care team and other providers of care as appropriate. § Provide ongoing monitoring of patient or client’s or family response to intervention; identifies deviations from expected outcomes and assesses change in patient status and reports proactively to attending provider. § Conduct thorough history intake during the admission process and throughout patient’s stay that includes but not limited to: Physical assessments, medication history, medical history, as well as in accordance with facility policies, procedures, or protocols. o Planning § Develop, coordinate and update an individualized plan of care, based on evidence based nursing practice, in collaboration with the patient and their family, the interdisciplinary health care team and other providers of care promoting continuity of care across the continuum. § Prioritize nursing interventions to achieve patient outcomes and goals. § Assists in the development and coordination of a comprehensive discharge and education plan, beginning upon admission, to meet the patient or client and their families’ needs, including health promotion, self-management, identifying and coordinating resources to promote continuity of care across the continuum, in collaboration with Care Coordinator/Discharge Planner. § Documents care and responses to care in a concise, and professional manner using the format designated within the Electronic Health Record appropriately. § Provides a supportive environment for communications and decision making by patients and their families o Intervention § Establishes and maintains a therapeutic relationship with patient or families. § Identifies, initiates and coordinates therapeutic interventions consistent with the legislated scope of practice and level of competence. § Implements prescribed interventions (e.g. diagnostic tests, medications) and performs independent nursing interventions (e.g. patient or client and family education). § Ensures the implementation, coordination and ongoing evaluation of the patient’s care plan. § Provides counseling and instruction that assists the patient or family in developing, improving, or regaining skills, social and community supports, promoting health and recovering from illness and disability. § Consults with the interdisciplinary team and other care providers to discuss changes related to patient, and modifies interventions within care plan. § Interprets medical information including diagnosis and treatment plans to patient or family and responds to patient or client questions and concerns. § Provides information and instruction to assist patients to understand health care practices and processes. § Utilizes educational material to support patients or clients understanding of their health care, health promotion, and or health maintenance, and the enhancement of their individual well-being. § Care is coordinated and delivered within the confines of available human, material and fiscal resources. § Performs appropriate treatments as ordered by physician/advanced practitioner in an accurate and timely manner. o Evaluation § Utilizes data obtained from the patient or client and their family and other health care providers to evaluate the plan of care including the patient or client’s responses to interventions and achievement of expected outcomes. § Uses data and evidence based data on best practice to evaluate nursing activities and interventions. § Documents complete, accurate and relevant data for every patient according to WMH policies, procedures, or protocols. § Provides detailed report to on-coming shift/staff. § Demonstrate competency o As Charge Nurse You Will: § Demonstrate Competency and Professional Responsibility in the Charge Nurse Role. § Supervise and evaluate care provided by direct care assistive and support staff. § Delegating tasks to staff and support staff § Hold others accountable and provide follow-up and feedback as warranted. § Manage admissions, discharges and transfers including necessary documentation. § Ensure smooth operation of the unit during your shift § Communicate with Director of Nursing § Act as a point of contact between doctors, nurses, and patients' families, communicating critical information clearly and promptly. § Responsible for finding shift coverage in absence of business hours support staff including completion of Unscheduled Absence paperwork. § Monitor medication administration and ensure all orders are followed properly. § Assign lunches and breaks for staff. § Makes unit assignments according to the needs of the patient and the capabilities of the staff. § Demonstrates knowledge of roles, responsibilities and rights of other nurses and other health care professionals. . o Professional Development, Leadership and Accountability § Practices within professional, legal and ethical standards and is accountable for his or her own actions and decisions. § Demonstrates professionalism in attitude, dress, and behavior. § Is present except for allowed time off, arrives prepared for shift and follows WMH policies and procedures. § Contributes to positive team functioning and supporting each other. Supports a respectful, team oriented workplace. § Develops cultural competence in order to provide culturally competent care. § Promotes a safe work environment by identifying and resolving potential risk issues. § Activity participates in professional development by attending education program offerings including additional education for WMH Trauma Level IV designation. Attends staff meetings and in-services as required by WMH and by accrediting agencies. § Participates in special project or assignments, hospital committee’s or outside projects. § Participates in an on-call rotation as per nursing policy as needed. § Performs all other duties as assigned or needed to meet the needs of the department/organization. § Demonstrates strong communication skills and adapt communication styles to meet the needs of specific patients, family members, or other health care team § Staff meeting attendance is required- at least 80% for each new year. § Attends at least 80% of staff meetings § Attends bi-annual nursing skills training . Ability to: • Lift, push, pull 50 pounds unassisted. • Maintain flexibility to adapt to a variety of workload assignments • Concentrate and be detail-oriented when planning and performing professional nursing care, resolving/dealing with patient/resident care problems while working with frequent interruptions. • Respond calmly and professionally when dealing with difficult or emergency situations. • Utilize time management concepts or maximize time effectively. • Communicate clearly and effectively, both verbal and written. • To problem solve in the delivery of patient/resident care, changes in patient/resident status and changes in staffing patterns • To perceive potential problems and respond appropriately and proactively. • To comprehend instructions in written, verbal, or diagram and care them out. • To modify care based on developmental/functional age of patient/resident as well as of patient/resident ethnic background. Complexity and Difficulty: • Resourcefulness in dealing with emergency situations • May be required to work on varied nursing units for varying periods of time to meet organizational needs. • Practices within scope of RN licensure. Minimum Qualifications: • Graduate of an accredited school of nursing • Associate Degree in Nursing; Bachelor’s degree in nursing preferred • At least one year of nursing experience in medical/surgical, emergency room or specialty clinical area preferred. • Current Montana state license as a registered nurse required, or by Compact state • American Heart Association BLS Certification required upon hire • ACLS and PALS required within 60 days of hire • TNCC and ENPC required within one year of hire. • Requires use of respiratory protection requiring fit testing and possibly medical clearance to wear a respirator. • Other certifications as may be required by specialty area. Working Conditions: • Normal patient/resident care environment in critical access hospital and emergency department • Utilization of computers and information systems • This job requires use of respiratory protection which will require respirator fit testing and medical clearance to wear a respirator. • Participates in on-call rotations for night, weekend and/or holiday support as needed. PHYSICAL DEMANDS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While filling this position, the employee must regularly lift and or move 10 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and depth perception.
